ATTENTION PARENTS

Please note that the nurse DOES NOT carry allergy medicine or any form of cold medicine. If your child needs daily allergy medicine or has symptoms of a cold, those medications must be taken at home (unless a prior arrangement has been made with the nurse). Please remind your child to take their allergy medicine EVERY DAY DURING ALLERGY SEASONS.


ANY MEDICATIONS THAT NEEDS TO BE STORED AT OR TAKEN AT SCHOOL MUST ALSO HAVE A SIGNED MD ORDER FORM. That form can be downloaded from this website or you can call or email my office.


**ATTENTION: ALL ATHLETES**

THE FREE SPORTS PHYSICAL CLINIC WILL BE HELD ON WEDNESDAY, MAY 23, 2018 FROM 2 PM TIL 5 PM IN THE DHS NURSES OFFICE. THIS WILL BE THE ONLY FREE PHYSICAL CLINIC UNTIL NOVEMBER, 2018.

Please check with Nurse Carloni at DHS or Nurse Sroka at DMS

if you are unsure of your last physical date.


INHALERS: if your child has asthma and needs to use an inhaler at school, signed permission from the MD and a parent must be on file with the nurse. DHS Students can carry their own inhaler IF allowed, in writing, by MD and parent. Please contact the nurse with questions or for forms. 

EPIPENS: if your child requires an Epipen for anaphylactic allergies, signed permission from the MD as well as parent, authorizing use, must be on file with the nurse. Students are allowed to carry their Epipen if they have signed authorization to do so, documenting that they have been trained on its proper use. This must be signed by parent and MD.

BLANK MEDICATION AUTHORIZATION FORMS CAN BE PICKED UP IN THE DHS MAIN OFFICE OR DOWNLOADED FROM THIS WEBSITE.


 **Attention** All students entering their Junior year are required by the State of CT and the Derby BOE to have on file with the nurse a current physical WITH UPDATED PROOF OF IMMUNIZATION. This physical must be completed and signed by an MD and dated after January 1 2017; sports physicals are not acceptable for this. The physical must be on the State of CT blue form, available from the nurse, main office or download here


Attention all college-bound students: 

Your college will require proof of immunizations for you to attend. You can request a copy from Nurse Carloni and it is recommended you keep a copy at home for future reference.
